Granite Falls Family Medical Care Center Is NCQA Certified

 

Granite Falls Family Medical Care Center is a National Committee for Quality Assurance (NCQA) certified Patient Centered Medical Home-Level III, a Diabetic Center of Excellence and a Coronary Artery/Stroke Center of Excellence.

NCQA’s Patient-Centered Medical Home (PCMH) 2011 is an innovative program for improving primary care. In a set of standards that describe clear and specific criteria, the program gives practices information about organizing care around patients, working in teams and coordinating and tracking care over time. The NCQA Patient-Centered Medical Home standards strengthen and add to the issues addressed by NCQA’s original program.

The Patient Centered Medical Home is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and in a culturally and linguistically appropriate manner. NCQA’s Diabetic Recognition and Coronary Artery Disease/Stroke Recognition Programs were designed to improve the quality of care that patients with diabetes, coronary artery disease and strokes receive by recognizing clinicians who deliver quality care for these patients. To receive recognition, which is valid for three years, Dr. Bujold and his staff submitted data that demonstrates performance that meets the Program’s key diabetic, stroke and coronary artery disease measures. These measures include eye exams, blood pressure tests, nutrition therapy, targeted numbers for lipid evaluations, patient satisfaction with their care and many other measurements. When patients receive this type of quality care they are much less likely to suffer from the complications of diabetes (blindness, kidney failure, amputations), strokes and heart attacks. To a person with these disease processes, the right clinician can make the difference between living with these diseases as opposed to suffering from them.
